Health Services Information
Immunizations and Health Assessments
- Complete immunization records and a health assessment signed by your family practitioner need to be received prior to or on the first day of school.
- Immunizations needed to enter kindergarten are:
5 DTaP vaccinations
4 Polio vaccinations
3 Hepatitis B vaccinations
2 MMR vaccinations
2 Varicella vaccinations OR verification of disease by a physician.
- If your child is exempt from any of the above vaccinations due to a medical condition or religion, the immunization waiver must be signed and received prior to or on the first day of school.
Medication Policy
- All prescriptions needing to be given at school MUST be in the original bottle with the pharmacy label on it. If you will need the medication at home and at school, you can ask the pharmacist for an extra labeled bottle.
- A prescription with the doctor's signature is required to be on file with the school nurse before the medication can be administered at school.
- A new prescription is required at the beginning of each school year for daily or as needed (i.e. inhalers, epi pens, etc.) medications you know will need to be given at school.
- If at some point during the year an antibiotic or new daily or as needed medication is prescribed and will need to be given at school, the same policy applies.
- Over-the-counter medications will only be given with parent/guardian approval.
- Cough drops, antiseptics, antibiotic ointment, and hydrocortisone cream will be administered if the medication form is filled out.
- Elementary and Middle School : Any medication and/or medical supplies MUST be kept in the nurse's office. This includes but is not limited to cough drops, inhalers, diabetic supplies, etc. Any and all of these items must be dropped off in the nurses's office first thing in the morning.
- High School : Students may carry and take their own prescription and over the counter medication unless the physician specifically orders the medication to be administered through the health room. Medication is to be carried in prescription bottles with the name of the medication and directions attached. Controlled drugs, such as Ritalin and narcotic pain medications, must be kept in a locked area of the office or health room and administered by the nurse or the designee.
Exclusion Policy
- Students with the following symptoms shall be excluded from school until they are SYMPTOM FREE FOR 24 HOURS
Temperature of 100 or higher
Diarrhea (2 or more loose or watery stools)
Vomiting
- The policy for students with head lice is as follows:
If live lice are found in a student's hair at school, they will be sent home and may not return to school until treatment with an antiparasitic drug is initiated.
If lice are found at home, treatment by an antiparasitic drug needs to be initiated and parents NEED to let the school nurse know. This will be kept confidential, but it is VITAL that you let your school nurse know.
If your child is a bus rider, they are not allowed on the bus until cleared by the nurse.
Before the student is allowed to return to class, a parent MUST bring the student to the nurse's office for the student's head to be checked. If no LIVE lice are found, the student will be admitted to class even if there are still some nits (eggs), but if there are live lice found, the student will be sent home. The student will need to have their head checked DAILY until n o nits are found.
Illness or Injury while at school
- If your child becomes ill while at school, a parent/guardian will be called to come and pick them up.
- There are some instances where a child is not feeling well but does not have a temperature. if this happens, your child will be evaluated and the nurse will determine the best course of action. That may include calling a parent to come pick them up, or it may be just letting the student rest and going back to class.
- A call to a parent is NOT made every time a student comes into the nurse's office. If you would like to see if your child was in the nurse's office on any given day, you may check Skyward Family Access. It will show any and all nurse office visits.
- If a student is injured at school, he/she is assessed the same way as above; a call may or may not be made home depending on the severity of the injury.
- If the nurse believes a student needs to be seen by a doctor, or evaluated, a parent will be called to pick up their child.
Contact Information
- Please make sure all contact information is kept current throughout the school year.
- In the event that a student needs to be picked up, every attempt will be made to first contact a parent. If the parent cannot be reached or will take more than 30 minutes to pick up their child, emergency contacts may be used.
- Students being sent home by the nurse will be waiting in the nurse's office. Parents need to go to the main office to sign their child out, and then they will be directed to the nurse's office.
Milk Allergies
- If your student has a milk allergy please notify the School Nurse. Water will be provided instead of milk for children with milk allergies.
Screenings
- Vision and hearing screenings are done in the fall for all Kindergarteners, 2nd, 4th, 6th, 8th, and 10 graders.
- If a child does not pass one or both screenings the first time, a second screening will be done about 2 weeks after the first.
- Parents are notified by letter ONLY if their child does not pass the 1st and 2nd set of screenings.
- If at any point during the year you are worried about your child's vision or hear, feel free to contact the nurse and ask for her to rescreen your child.
