How to Enroll

  • Enrollment 
    To enroll in De Soto Parents as Teachers, please click on the enrollment form link, complete the form and submit it. The PAT Coordinator will contact you to schedule a Welcome to PAT/screening visit with your family. Once you are paired up with a parent educator, a team member will contact you to schedule your first personal visit.

    Para inscribirse en De Soto Parents as Teachers, haga clic en el enlace del formulario de inscripción, complete el formulario y envíelo. El coordinador de PAT se comunicará con usted para programar una visita de Bienvenida a PAT/detección con su familia. Una vez que esté emparejado con un educador de padres, un miembro del equipo se comunicará con usted para programar su primera visita personal.

    In order to match you with a Parent Educator, we would appreciate your best estimate of when you will be available for visits until your child is three (3) years old. On the enrollment form, please mark the time of day that you will be available for ongoing visits.
