New Student Enrollment Process
Welcome to Mill Creek Middle School! We hope you will find this information helpful as you enroll your student in our building!
The first step in the enrollment process is to complete an enrollment packet which can be found here. You can pick up a paper copy from MCMS or print out the packet complete it at home, and return it to the school.
In order to enroll in USD 232, you must provide the following documents:
- Completed Enrollment Packet
- Proof of residency
- Birth certificate
- Individalized Education Plan (IEP) or Section 504 Plan, if applicable
- Grades/schedule from previous school, if applicable
Once we have received all the documenation listed above, a counselor will reach out to you and your student to go over the schedule, and give you any additional information. We know transitioning to a new school can be difficult for some students, especially in middle school. Part of our committment to you is to insure all new students and families feel welcome at MCMS.
