Health Conditions

The district will develop individualized healthcare plans for students diagnosed with diabetes.  The individualized healthcare plan will be developed through collaboration between the diagnosing healthcare provider, the parents/guardians, the student, and the school nurse.  Parents/guardians will provide the school nurse orders from the diagnosing healthcare provider, discuss the student's current health and management, and provide all necessary supplies for management.  When developing the individualized healthcare plan, the school nurse will take in consideration the age of the student, the date of diagnosis, and the student's self-management skills for independence in care.  If necessary, the school nurse will request the parents/guardians fill out the diabetes health intake form.

The district will develop individualized healthcare plans for students diagnosed with seizures.  The individualized healthcare plan will be developed through collaboration between the diagnosing healthcare provider, the parents/guardians, the student, and the school nurse.  Parents/guardians will provide the school nurse orders from the diagnosing healthcare provider, discuss the student's current health and management, and provide all necessary supplies for management.  When developing the individualized healthcare plan, the school nurse will take in consideration the age of the student, the date of diagnosis, and the student's self-management skills for independence in care.  If necessary, the school nurse will request the parents/guardians to fill out a seizure health intake form to develop the individualized health care plan.

The district will develop individualized healthcare plans for students diagnosed with asthma.  The individualized healthcare plan will be developed through collaboration between the diagnosing healthcare provider, the parents/guardians, the student, and the school nurse.  Parents/guardians will provide the school nurse orders from the diagnosing healthcare provider, discuss the student's current health and management, and provide all necessary supplies for management.  When developing the individualized healthcare plan, the school nurse will take into consideration the age of the student, the date of diagnosis, and the student's self-management skills for independence in care.  If necessary, the school nurse will request the parents/guardians to fill out an asthma health intake form to develop the individualized health care plan. 

Asthma Health Intake Form

The district will develop individualized healthcare plans for students diagnosed with severe allergies.  The individualized healthcare plan will be developed through collaboration between the diagnosing healthcare provider, the parents/guardians, the student, and the school nurse.  Parents/guardians will provide the school nurse orders from the diagnosing healthcare provider, discuss the student's current health and management, and provide all necessary supplies for management.  When developing the individualized healthcare plan, the school nurse will take in consideration the age of the student, the date of diagnosis, and the student's self-management skills for independence in care.  If necessary, the school nurse will ask the parents/guardians to fill out a severe allergy and anaphylaxis health intake form to develop the individualized health care plan.
